Report/Student List Screen

 Purpose

The Report/Student List screen is used to view students added to your school list, and run report's for the students in the list. You can also view a students history and remove a student from your list.

 Information Provided

This section is for viewing the students contained in a selected student list. Information contained within the client list table is described below.

Column Name

Description

Select

If you wish to generate reports or delete a select number of students from your student list you use the Select column along with the  Print  or  Delete  buttons. Place a checkmark in the Select box beside the student(s) you wish to select. You can select as many students as you like at one time by checking multiple students. Next click on either button.

Last Name

This is the last name for the student. It is also used as a link back to the Student Immunization History screen. Click on this link and you will be taken to the history screen for the student.

Middle Name

This field displays the middle name for the student.

First Name

This field displays the first name for the student.

Birth Date

This field displays the date of birth for the student.

Date 3231 Generated

This field displays the most recent date a 'Complete' Form 3231 was generated for the student using the  Student Imm Status Rpt  or  Periodic Imm Status Rpt  buttons. If the field is blank, the student is not 'Complete' for school attendance, or a 'Complete' Form 3231 has not been generated for the student.

 Functionality

This section provides options for working with your students list. Several reports options are available. Links are available for reviewing a student's shot record, and you may delete selected client(s) from the school list.

Report Button

Description

 Student List 

This report displays the name and date of birth for each student on the student list sorted alphabetically by last name.

 Student Imm Status Rpt 

The Student Immunization Status reports are a group of three reports generated for a student list. Immunization Status Summary - displays a list of students who are 'Complete' for school attendance and a list of students who are 'Incomplete' for school attendance. Immunization Status Form 3231 - displays a Form 3231 for all students who are 'Complete' for school attendance. Immunization Status Immunization History - displays an Immunization History report for all students who are 'Not Complete' for school attendance.

 Periodic Imm Status Rpt 

The Student Immunization Status reports are a group of four reports generated for a student list. Periodic Report Summary - displays a list of students who are 'Newly Complete' for school attendance, a list of students whose Form 3231 will expire within 30 days, and a list of students whose Form 3231 is currently expired. Periodic Report Form 3231 - displays a Form 3231 for all students who are 'Newly Complete' for school attendance. Periodic Report Status Expired - displays an Immunization History report for all students whose Form 3231 is currently expired. Periodic Report Status Expire less than 30 Days - displays an Immunization History report for all students whose Form 3231 will expire within 30 days.

 Print 

The Print button generates the same three reports as the Student Immunization Status reports; however, the reports are only generated for students who have been selected from the student list.

Button/Link

Description

Index: A ... Z

Directly above the student list, the letters of the alphabet from A to Z are displayed. These letters are links that allow you to jump through a large list of students to locate last names that start with a selected letter.

[Last Name]

On the student list, click on the last name of the student you wish to view. The Student Immunization History screen be displayed.

 Select All 

This button is used to select all students in your list.

 Unselect All 

This button is used to unselect all students in your list.

 Delete 

This button is used to delete selected student(s) from your list. Select the student(s) you want to delete, then click the  Delete  button.

 Cancel 

Click on the  Cancel  button to go back to the Manage List screen.
