Client Reports Screen

 Purpose

The Client Reports screen exists so that, after finding a client in the registry, a user may generate and/or print several different types of reports for that client.

 Functionality

The Client Reports screen is accessible from either the Edit Client screen or the Immunization History screen, and cannot be accessed directly from the Menu Panel/Status Bar. A client must first be found using the Manage Client/Immunizations screen. Then, after accessing either of the above two screens, the Client Reports screen can be reached by clicking on the  Reports  button.

At the top of the screen there is a client information header displaying various relevant information, the same as in the Immunization History screen. This can be used to verify the client being reported on.

Report Name

Description

Vaccine Administration

This report displays demographics, contact information, immunization history, as well as immunizations available.

Complete Immunization

This report displays demographics, registry data, contact information, as well as a detailed immunization history.

Immunizations Needed

This report displays demographics, contact information, immunization history, as well as immunizations needed according to the clients tracking schedule.

Form 3231

This report displays the immunization history for a child attending any school or daycare facility.

If your computer does not have Adobe Acrobat Reader version 5.0 or greater installed, please see the " **REPORT VIEWING REQUIREMENTS** " notification on the Client Reports screen, or contact your administrator.
